Registration form must be received at least two weeks prior to the scheduled exam
Registration:
- Written Exam Registration Form for SD, AG, AS & AE Module
- Effective Jan 2007: Performance Registration Form for SD, AG, AS & AE Module
- with the Performance Registration form, fill out the Rights and Responsibilities aggreement
Checklist:
